Assumption of Risk, Waiver of Liability, and Indemnification Agreement
In consideration of being allowed to participate in any way in the MHMA Wrestling Competition, the undersigned acknowledges, appreciates, and agrees that:
- Assumption of Risk:I understand and acknowledge that wrestling is a physically demanding and potentially dangerous sport, which involves inherent risks including but not limited to falls, contact with other participants, and the risk of injury, paralysis, or even death.
I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation.
- Waiver of Liability:I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless MHMA, its officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used for the activity ("Releasees"), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
- Indemnification:I agree to indemnify and hold harmless the Releasees from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of Releasees or otherwise.
- Medical Treatment:I consent to receive medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during this activity.
- Compliance with Rules:I agree to comply with the stated and customary terms and conditions for participation. If I observe any unusual significant concern in my readiness for participation and/or in the program itself, I will remove myself from participation and bring such attention to the nearest official immediately.
